At Evergreen Place Assisted Living, we are committed to helping our residents meet their individual needs. We value each individual that comes to Evergreen Place, and to ensure only the best experience, we have designed our mission, values, and management philosophy around our senior community.

We are committed to assisting our residents to pursue individually satisfying lifestyles and to giving their families peace of mind by providing quality assisted living services in a secure, supportive, and home-like setting, as well as cultivating a valued relationship with the local community.

Independence
Personal choice and options are key to independent living. Evergreen Place seniors personally choose the amount and type of assistance they would like from our caring staff. Our assisted living facility is designed around our residents – all residents decide on their own day-to-day activities, spending their time as they please. Residents of our senior community enjoy independence through choice.
Individuality
Each resident at Evergreen Place brings unique qualities to our senior community. As a community, we value these qualities, allowing our residents to make their care as individualized as possible, tailoring it to meet to their personal needs.
Sense of Belonging
At Evergreen Place, we work hard each day to create a safe, satisfying, and fun community for our residents. Our seniors can interact with one another through planned activities, quiet conversation or during meals. Residents form strong relationships with one another, as well as with our caring staff.

Because Evergreen Place is designed around our residents' needs, we work hard to meet those needs in a respectful and dignified manner. We honor our seniors' requests for privacy, as well as the choices they make.

At Evergreen Place, we are fully committed to supporting and maintaining the dignity, respect, choice, independence, self-determination, and wellness of our community. As a result, our staff upholds the following philosophical principles when it comes to caring for and working with our residents.
Choice and Shared Responsibility
Our residents remain our central focus. We establish a partnership with the resident and their family, allowing them to make choices about their care. The resident is always a part of the decision-making process in order to provide them with the utmost control. Our residents can maintain their own independence, requesting as much or as little assistance as needed.
Managed Risk
We do not limit the choices our residents' and their families make. We realize that some choices may bring inherent risks; however, we address these risks with the residents and their families, allowing them to make the final decision.
Service Management
In order to provide the best, personalized elder care for our residents, we conduct an individualized assessment of residents' service needs and preferences. Residents and their families are looked to for input, and from there, we decide on the level of care needed, together.
Job Responsibilities and Training
Evergreen Place's highly qualified and caring staff works hard to meet the individual needs of each resident. We hire only the best, most dedicated staff who will work with our residents and their families to make senior living a pleasant experience.
Each member of the Evergreen Place team truly contributes to the quality care we provide for our residents. Our residents benefit from a team whose first priority is our residents' care, no matter the task. This approach helps empower our team members, encouraging them to provide timely, quality services to those who call Evergreen Place home.

Evergreen Place employs six coordinators who represent the interdisciplinary service areas of Administrative Services, Building Services, Food Services, Marketing, Resident Services, and Wellness, and a Director who oversees all operations. Our residents benefit from the combined experience of these managers totaling over 95 years in the senior housing industry, including Independent Living, Assisted Living, and Skilled Nursing. Following are brief descriptions of each of these areas.
Director. The Director at Evergreen Place is responsible for the overall operation of the facility. This responsibility includes making sure the facility operates in accordance with all company, state, and federal rules and regulations.
Administrative Services Coordinator. Our Administrative Services Coordinator serves in the capacity of office manager and is responsible for handling most of the clerical and accounting aspects of our facility.
Building Services Coordinator. At Evergreen Place, we want to provide a clean, comfortable home for our residents. The Building Services Coordinator works diligently, along with two housekeepers, to ensure the facility is well-maintained and clean for our residents. Residents can also look to this coordinator for help with tasks, such as hanging artwork or driving them to appointments or activities in the facility's van.
Food Services Coordinator. Our Food Services Coordinator provides residents at Evergreen Place with a variety of delicious meals that meet individual nutritional needs and food preferences. This coordinator also supervises cooks and dining room servers who provide our residents with a restaurant-style meal service three times each day.
Marketing Coordinator. Our Marketing Coordinator works with residents during the move-in process. This coordinator also builds relationships throughout the community to help bring awareness to the community of all Evergreen Place has to offer.
Resident Services Coordinator. Our residents benefit from a variety of activities and social services organized by the Resident Services Coordinator. This coordinator works to implement a schedule of activities designed to meet the emotional and physical needs of each resident.
Wellness Coordinator. Our residents' care is overseen by our Wellness Coordinator, who is a Registered Nurse in the State of Illinois . Along with our coordinator, a part-time nurse and resident caregivers are available to our residents to provide customized services, including assistance with medication, showering, or dressing.
